At Check-In you must have the following:

 Official Team roster signed by your Club RegistrarCompleted and signed Medical Release FormUSYSA issued Player cards
 
There will be NO exceptions to these rules.

Medical Release Form

The tournament registration process has been developed to provide all of your registration, payment, team roster, bracket information, game schedule and game scores online. Checks will be accepted as well. Begin the registration process by completing a simple online form and creating your own User ID and Password. You may take as much time as you need to complete your application. You have the ability to return at any time and work with your application until you are ready to submit payment. Please note that the application is not deemed submitted until it has been submitted AND your registration fees have been paid. Team representatives are REQUIRED to check-in their team during the listed times -- there will be NO Saturday morning check-in unless prior arrangements are made with the TournamentDirector.

We are asking for all teams registering to identify their current playing level. Teams will be placed into the flights of competition by the tournament bracket committee. Please provide sufficient information in the "Team History" section of the online registration system to allow the correct placement.
